Please download and complete all the forms below and return completed forms to the front desk, any staff member, or email to Christine@phoenixvilleseniorcenter.org. If you have questions, please call us at 610-935-1515 or ask at the front desk.
After we put your information in our computer, you will check in at the computer each time you come to the center.
If you would like to know more about our volunteer opportunities, please speak to any staff member for more information.
Finally, if paying for your dog or cat’s food is a financial hardship, please consider filling out the “Henry’s Fund Pet Food Selection Form.” Pet food is usually delivered quarterly to the Center. Speak to any staff member for more information.
